March 21, 2009 was the deadliest day in the history of the Oakland Police Department: four of its officers were killed by a single perpetrator during what began as a routine traffic stop. In the aftermath, the OPD commissioned an independent review of the incident by the Institute for Public Research. The report praised individual officers for "high levels of courage and disipline" but found that at various points throughout the two-hour incident, protocol was not followed and that there was a "lack of overall incident coordination."
